3. You can scroll down to complete an online form with your name, contact information, address and retailer’s information. Then submit.
4. You must then print the electronic form and mail-in along with the original UPC barcode cut from the BIC package, and the original store cash register receipt dated 7/21/19-9/7/19 with the product purchase price circled.
