How to apply:

- Provide your gender, age, the closest major city to you, how many children under 18 you have living at home and their ages.
- Specify if your child helps decide their before or after school care programs, you and your child’s current involvement in childcare programs, the types of programs you would consider (nanny, YMCA, Tutor Time etc.) and related information.
- Also provide information on your marital status, living arrangements, income and more.
- Provide your name, contact information and zip.
