How to request you free package:

- Enter your name and email address, then add your hospital/ facility’s name and shipping address.
- Note who the package should be directed to.
- Upload a photo of your hospital ID card.
- Note how many co-workers you would like to gift and if you are a part of the Vasanti Community.
- Hit “Submit” to request your free package while supplies last!
